- INE (National Electoral Institute) card or identity letter of the applicant and the accompanying responsible party.
- CURP (Unique Population Registry Code).
- Two proofs of address from different services.
- Two personal references with their address and phone number.
What to Expect During Your Visit:
During your appointment at the Urban Health Center B, located at the intersection of Puerto Progreso and Puerto Anzio S/N, in the Portal del Roble neighborhood, you will:
- Complete a Psychological Evaluation (ASSIST-POSIT).
- Undergo an interview to verify your data through a socioeconomic study.
- Sign a consent form.
